Benefit Summary – House Officers

Effective July 1, 2014

Listed below is a summary of the core benefits available for House Officers hired to work 20 hours or more per week, except where noted part-time employee benefits will be pro-rated. (Below is a brief summary that should be used as a reference only. The actual contracts will prevail.)

This document provides a general overview of company benefits. Refer to the Summary Plan Descriptions (SPD) and policies for full details, benefits, restrictions and requirements. In the event of a conflict between this document and the SPDs and/or company policies, the SPDs and policies govern. All plan coverage descriptions assume in-network utilization.

 

Paid Time Off

  • Twenty (20) paid days off per residency contract year (includes continuing medical education days)
  • Paid time off runs from July 1 to June 30 and is front loaded
  • Paid days off may not be carried over to any subsequent Agreement. Unused paid days will not be reimbursed

Sick Time

  • One (1) sick day per month to a maximum of 12 days per residency contract year
  • Sick days may not be carried over to any subsequent Agreement
  • Unused sick days will not be reimbursed

Tuition Reimbursement

  • Employees averaging 32+ hrs each wk during the semester in which the course is taken, may receive 100% tuition cost up to $1600 per school year
  • Employees averaging 20-31 hrs per wk during the semester in which the course is taken, may receive 50% of tuition cost up to $800 per school year
  • All other eligibility requirements must be met

401(k) Investment Opportunities

  • Eligibility: All full-time, part-time and per diem employees are eligible after 30 days of employment.
  • 1% – 75% pre-tax contribution up to IRS limits. Up to 10% after tax
  • Vesting Schedule – 5 years – 20% each year
  • Employer match
  • 50% of before-tax contribution up to 6% of eligible compensation
  • Must be actively employed on 12/31 to receive match
  • Must work 1,000 or more hours during the plan year

Employee Stock Purchase Plan

  • Purchase Tenet Stock at 5% discount
  • Must be budgeted to work 20 hours per week and have completed 30 days of service
  • Enrollment opens 30 days prior to start of calendar quarter and open for 15 days: example (June 1-15 for purchase July 1)

Medical – 1

Administrators: Medical – Varies by location. Prescription – CVS/Caremark HSA – Fidelity. HRA – Hewitt. Tenet shares the cost of the plan with employee.

  • Two medical plan options: Health & Savings Plan, Health & Reimbursement Plan
  • Both medical plans provide preventive care coverage at 100%* and include a prescription drug plan
    • The Health & Savings Plan includes a personal, tax-advantaged Health Savings Account (HSA)
    • The Health & Reimbursement Plan includes a company-funded Health Reimbursement Account (HRA)
    • Both accounts are used to pay qualified health care expenses. Both accounts’ balances roll over year after year.
    • HSA features:
      • You may contribute to account. Deposits, interest and withdrawals are free of federal taxes when used to pay qualified expenses
      • Account is yours to keep if you change medical plans or leave Tenet
      • Any health incentive (up to $800) earned in 2014 is deposited into HSA in 2015
    • HRA features:
      • Tenet makes a company contribution: $300 employee only or $600 employee + spouse/child/family**
      • Any health incentive (up to $800) earned in 2014 is deposited into HRA in 2015
      • Unused balance is lost if you change plans or leave Tenet

* Assumes in-network utilization. ** Prorated for new employees.

 

Dental – 1

Administrator: CIGNA. Tenet shares the cost of the plan with employee. Two plan options:

  • Comprehensive: 100% coverage for preventive care, 80% for basic, 50% for major care; $25 per person deductible
  • Preventive: 100% coverage for preventive care only; no deductible

Vision – 1

Administrator: VSP.

  • $10 exam co-pay▪ $15 materials co-pay (frames and lenses or contacts), up to allowed maximums

401(k) – 3

Administrator: Fidelity.

  • Employees make pre-tax contributions through payroll deductions
  • Variety of investment options from which to choose
  • Contribute 1% – 75% of eligible compensation; annual maximum $17,500 (or $23,000 if age 50 or older)
  • Tenet currently has a discretionary match of 50% of up to 6% of eligible pay based on contribution
  • Company contribution fully vested after 5 years

Wellness Program – 4

Administrator: Tenet. No cost to employee.

  • Employees and eligible spouses may earn a health incentive by meeting program requirements
  • Health incentive earned in 2014 will be paid in 2015
  • Wellness activities promote physical, community, environmental, learning, financial, and emotional wellness

Tenet Personal Health Team & Personal Assistance Program – 5

Administrator: CIGNA. No cost to employee.

  • Free, confidential coaches, counselors, dieticians, and referrals provide assistance with life and wellness issues, chronic condition management, and mental health and substance abuse services

Basic Employee Life Insurance – 6

Administrator: Unum. No cost to employee.

  • Automatic coverage of one times annual base pay, up to $50,000

Basic Employee AD&D Insurance – 6

Administrator: Unum. No cost to employee.

  • Automatic coverage of one times annual base pay, up to $50,000

Supplemental Life Insurance – 1

Administrator: Unum.

  • Optional coverage for employee at 1, 2, 3, 4, 5 or 6 times annual base pay, up to $2.5 million
  • Optional coverage for spouse at $5,000, $10,000, $25,000, $50,000 or $100,000 (max. 100% of employee supplemental)
  • Optional coverage for child(ren) at $5,000, $10,000 or $25,000 (maximum 100% of employee supplemental)

Supplemental AD&D Insurance – 1

Administrator: Unum.

  • Optional coverage for employee at 1, 2, 3, 4, 5 or 6 times annual base pay, up to $500,000
  • Optional coverage for spouse at $25,000, $50,000 or $100,000
  • Optional coverage for child(ren) at $10,000 or $25,000

Manager’s Disability Insurance – 6

Administrator: Unum. Includes both short-term and long-term provisions. Two plan options:

  • Standard: After 30-day elimination period, plan pays 50% of base pay (maximums apply)
  • Standard Plus: After 30-day elimination period, plan pays 60% of base pay (maximums apply)

Health Care Spending Account – 1

Administrator: Hewitt. Plan not available to Tenet’s Health & Savings Plan medical participants.

  • Employees make pre-tax payroll contributions of $130 – $2,500 annually to use for qualified health care expenses
  • Funds must be used annually (use it or lose it)
  • Total annual funds available as of coverage effective date

Dependent Day Care Pending Account – 1

Administrator: Hewitt.

  • Employees make pre-tax payroll contributions of $130 – $5,000 annually to use for qualified day care expenses
  • Funds must be used annually (use it or lose it)
  • Funds available as account balance allows

Employee Stock Purchase Program – 7

Administrator: Fidelity.

  • Employees purchase Tenet common stock, each quarter, at 5% discount through payroll deductions
  • 1% – 10% of base salary ($25,000 annual maximum)
  • Employees must hold shares for at least one year

Legal – 1

Administrator: Hyatt Legal.

  • Low cost legal services for employees and family through a network of more than 11,000 attorneys

Long-Term Care Insurance – 6

Administrator: Unum.

  • Coverage for assisted living, long-term care facility, or in-home care services when an individual is unable to function independently due to covered disability or chronic illness
  • Available for employee, spouse, domestic partner, parents, step-parents, and/or grandparents and is portable

Business Travel Accident Insurance – 1

Administrator: Hartford. No cost to employee.

  • Additional life and AD&D benefits payable if employee death or serious injury occurs while traveling on company business
